State of Illinois Health Requirements for the 2020-2021 School Year
Requirements by Grade Level:
- Please note that all Physical Exams, Immunizations, and Vision Exams are due by October 15th of the school year
Preschool Students:
- Physical exam (parents please complete the yes/no questions on the back and sign and date)
- Complete immunization record signed by a doctor: (4)DTap, (3)Polio, (4)Hib, (3)HepB, (1)MMR, (1)Vericella, (4)Pneumococcal
- Birth Certificate
Kindergarten Students:
- Physical exam (parents please complete the yes/no questions on the back and sign and date)
- Complete immunization record signed by a doctor: (5)DTap, (4)Polio, (4)Hib, (3)HepB, (2)MMR, (2)Vericella, (4)Pneumococcal
- Eye Exam on IL form (must be dated within one year of the first day of school)
- Dental Exam on IL form due May 15th of school year (dated within 18 months of May 15th deadline)
- Birth Certificate
Second Grade Students:
- Dental Exam on IL form due May 15th of school year (dated within 18 months of May 15th deadline)
Sixth Grade Students:
- Physical Exam (Sports physical NOT acceptable; IL Certificate of Child Health Exam form required) Parents, please complete yes/no questions on the back and sign and date.
- Complete immunization record signed by a doctor, including: (1)Tdap and (1)Meningitis
- Dental Exam on IL form due May 15th of school year (dated within 18 months of May 15th deadline)
Ninth Grade Students:
- Physical Exam (Sports physical NOT acceptable; IL Certificate of Child Health Exam form required) Parents, please complete yes/no questions on the back and sign and date.
- Complete immunization record signed by a doctor
- Dental Exam on IL form due May 15th of school year (dated within 18 months of May 15th deadline)
Twelfth Grade Students:
- Proof of receiving (2)Meningitis: First dose received on or after the 11th birthday and second dose received at least 8 weeks after the first. Only one dose is required if the first dose was received at 16 years of age or older.
